Patient Centered Care Policies

At Patient Centered Care, we have a goal to have an office that runs smoothly and does not have patients sitting in the lobby waiting for long periods of time and does not have the nurse practitioner (NP) rushing. We cannot do this without your assistance. When you call to schedule an appointment, we ask you to please use this list to help you determine what type of visit you need and how long you need with the NP. Please make the person scheduling your appointment aware of ALL problems you wish to discuss at your appointment. We encourage you to schedule your visit for as long as you desire your visit to be. The staff will work with you to determine how much time you will need to address your concerns. You will have the NP’s undivided, unhurried, and focused attention for as long as you have scheduled.

We ask that you please schedule all appointments. We do not accept walk-ins to prevent patients from waiting long periods of time.If you need a same day visit, we ask that you call before 10 AM, and we will try to accommodate you. If you must cancel your appointment, please cancel at least 24 hours in advance. If you are late for your scheduled appointment, you will be asked to reschedule your appointment. If you miss your appointment or do not cancel it at least 24 hours in advance, there will be a $25 charge to your account. Thank you for your help and participation in our scheduling process.
