Patient Centered Care Appointment & Policies

  • At Patient Centered Care, we have a goal to have an office that runs smoothly and does not have patients sitting in the lobby waiting for long periods of time and does not have the nurse practitioner (NP) rushing. We cannot do this without your assistance. When you call to schedule an appointment, we ask you to please use this list to help you determine what type of visit you need and how long you need with the NP. Please make the person scheduling your appointment aware of ALL problems you wish to discuss at your appointment. We encourage you to schedule your visit for as long as you desire your visit to be. The staff will work with you to determine how much time you will need to address your concerns.
  • You will have the NP’s undivided, unhurried, and focused attention for as long as you have scheduled.
  • All co-payments are due at the time of the appointment. We accept cash, check, credit cards, venmo. A $25 service charge will be applied for all returned checks
  • We ask that you please schedule all appointments. We do not accept walk-ins to prevent patients from waiting long periods of time. If you need a same day visit, we ask that you call before 10 AM, and we will try to accommodate you. If you must cancel your appointment, please cancel at least 24 hours in advance. If you are late for your scheduled appointment, you will be asked to reschedule your appointment. If you miss your appointment or do not cancel it at least 24 hours in advance, there will be a $30 charge to your account. Thank you for your help and participation in our scheduling process.

  • Prescriptions are filled within 48hrs of receiving request from pharmacy.

IF YOU HAVE

CHEST PAIN, FAINTING,  SHORTNESS OF BREATH, LACERATIONS, HAVE BEEN IN A MOTOR VEHICLE ACCIDENT/WORKERS COMPENSATION ACCIDENT;
PLEASE GO TO THE
EMERGENCY ROOM OR URGENT CARE.

Acute/Follow-up Appointment:
Each visit 30-60 minutes. Established patients with 1 problem only. Examples: Urinary Tract Infection, Cold, Sinusitis, Flu, Ear Pain, Sore Throat, Nausea & Vomiting, Diarrhea, Constipation Hemorrhoids, Allergy Flare, Rash, Eye Infection, Fever, Insect Bite, Nosebleed, Stable Hypertension Follow-up, Refill for one medication, Follow-up for 1 stable ongoing health problem.

New Patient I/ Extended Visit I/ or Physical (age over 40) Appointment:
Each visit 60-90 minutes. New patients with 1 acute/follow-up problem, established patients with 2 acute/follow-up problems, or physicals for established patients whose age is over 40. Other problems that need this type of visit for an established patient are: Breast Pain/Lump, Vaginal Itching/Discharge, Musculoskeletal Injury, Rectal Bleeding, Asthma Exacerbation, Back Pain, Animal Bite, Burns, Eye Trauma, Refills for several medications, Referrals, Unstable ongoing health problems.

New Patient II (2 problems)/Extended Visit II (3 problems)/ or Physical (age 40 and older) Appointment:
Each visit 60-90 minutes. New patients with 2 acute/follow-up problems or 1 extended visit I problem, established patients with 3 acute/follow-up problems, or physicals for new patients whose age is under 40 or physicals for established patients whose age is 40 or greater. Other problems that need this type of visit for an established patient are: Abdominal Pain, Headache, Dizziness, New Psychiatric Problems/Symptoms, and Abuse.

New Patient III (3 or more problems) or Extended Visit III (>3 problems) Appointment:
New Patient III (3 or more problems) or Extended Visit III (>3 problems) Appointment: Each visit 1 hour 30 minutes. New patients with 3 acute/follow-up problems or 2 extended visit I or II problems, new patient physicals for women age 40 or greater, or established patients with greater than 3 acute/follow-up/extended visit I/extended visit II problems. Use also for nutrition/weight loss counseling for women AND men.
